Frequently Asked Questions
What is longevity medicine, and how it is different from primary care?
Longevity medicine is a specialized clinical practice focused on early detection of disease and disease risk through advanced testing as well as application of evidence-based treatment and prevention interventions to keep you as healthy as possible for decades to come. Longevity medicine cares about your 30 or 40 year risk of disease, not just the next decade. The work happens decades before disease shows up, when the menu of interventions is widest. Primary care is structured to catch disease and manage it with prevention aimed at the population level, not you as an individual. Longevity medicine is structured to prevent disease and optimize the systems that decide how well you live in the last decades of your life — cardiovascular, metabolic, hormonal, cognitive, and musculoskeletal.
At Empower Wellness Medical Clinic, the Executive Longevity Telehealth Membership delivers that work through one physician caring for you as a whole person and extensive advanced testing personalized to your health and priorities to develop your comprehensive, forward looking prevention plan. The model is direct pay. No insurance billing for included services. Members are encouraged to carry insurance or a Healthshare plan for things outside the practice’s scope (emergencies, hospitalizations, surgeries). For labs and imaging beyond the included annual panel, we have negotiated discounted cash prices through partner labs, or you can also use your insurance if you prefer.
Frequently asked questions
-
Excellent medical care, longevity medicine or primary care, requires more depth, time, and continuity that a 10-minute insurance-billed visit cannot deliver. The membership model is what allows the to to provide excellent, comprehensive care. The membership based direct care model allows us to deliver this care with a predictable monthly fee.

We offer two membership options based on your healthcare needs and location.
Concierge Primary Care Membership
Ideal for patients who live near our practice, or are able to travel to our office for in-person care. This membership provides comprehensive primary care services for your day to day care, including sick visits, chronic disease management, preventive care, and ongoing wellness support. This does not include longevity care.Executive Longevity Telehealth Membership
Our telehealth-only longevity membership is designed for patients seeking a proactive, wellness-focused approach to optimizing health, performance, and longevity above and beyond primary care services. This membership does not include primary care and is best suited for patients who already have an established primary care provider for routine medical care and urgent health concerns. Not sure which membership is the best fit? Contact our team, and we’ll help guide you based on your goals and location.
-
The membership itself is direct pay and does not bill insurance. For anything that isn’t included in your membership — additional labs, imaging, specialist referrals, hospitalizations — you can use your insurance as you normally would. The membership is designed to work alongside insurance, not replace it.
-
Yes. The membership is not insurance and does not replace it. We recommend maintaining traditional health insurance or a healthshare plan to cover anything outside the practice's scope — hospitalizations, surgeries, emergencies, and specialist care. Many members find healthshares a cost-effective option for catastrophic coverage.
-
Many members use HSA, HRA, or FSA funds toward the membership, since these accounts align with the tax code's definition of qualified medical expenses. We recommend paying annually rather than monthly if you are using HSA funds. These accounts can also be used for additional labs, imaging, and prescriptions. We are not able to provide tax or legal advice — consult your tax advisor for guidance specific to your HSA or FSA plan and your situation.
-
The membership works well for Medicare-eligible adults who want longevity or concierge care with a physician they can actually access. Membership fees are paid directly to the practice. Medicare coverage remains in place for everything outside the membership: specialist visits, testing performed outside our office, and hospitalizations. Labs prescribed by Dr. Stehling can be billed through an independent lab affiliate that takes Medicare. The practice can still refer, prescribe, and coordinate care alongside your Medicare coverage.
-
Anything beyond the included membership services can be ordered through partner labs, imaging centers, your usual pharmacy or a wholesale pharmacy at discounted cash prices. You can pay cash, use HSA or FSA funds, or use your insurance — whichever is most convenient for that specific test or prescription. Where possible, we will tell you the cash price upfront so you can compare.
-
Executive Longevity Membership visits are quarterly via telehealth (video). You can schedule online, by secure messaging, or by phone. Follow up visits are typically scheduled at the end of a visit.
Forms are completed online in advance, and visits start on time. Initial labs must be done 2 weeks prior to your initial visit to ensure we have results for your first visit.
If you need a visit beyond your included quarterly visits, visits can be scheduled with Dr. Stehling for an additional visit fee.
-
Concierge visits take place in-person or via telehealth (video visit). The first visit and one visit annually must be in-person. Subsequent visits can be in-person or via telehealth, whichever is most convenient for you.
Forms are completed online prior to the visit.
There is no extra charge for visits. Your membership fee covers all visits at Empower Wellness Medical Clinic. Specialized testing that takes place at the clinic like VO2 max testing or resting metabolic rate testing has an additional fee.
-
Patients can easily connect with our clinic through Spruce, a secure, HIPAA-compliant messaging app that allows you to send messages, photos, and videos directly to our team. Of course, we’re also available by phone whenever you need us.

Concierge Primary Care Membership: 707-760-7249
Messages are answered within two business days, though most member questions receive a same-day response. Scheduling, insurance, and general office questions are handled by our administrative team, while medical questions are addressed directly by Dr. Stehling.

If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room immediately. Examples of emergency situations may include chest pain, stroke-like symptoms, severe abdominal pain, difficulty breathing, significant injuries, or possible miscarriage.
Our memberships are designed to support preventive, primary, and longevity-focused care, but they do not include emergency or hospital services. For this reason, we recommend maintaining health insurance or a healthshare plan to help cover hospitalizations and emergency care.
If hospitalization is needed, we are happy to coordinate with your care team by providing relevant medical records as appropriate. For Concierge Primary Care members, we also work closely with hospital care teams to help support a smooth transition back home and ongoing primary care follow-up after discharge.
-
The total patient panel is intentionally capped at 400 members. A traditional primary care panel is 2,000 to 3,000 patients. A longevity or concierge practice cannot function at that scale — the visits are longer, the testing is deeper, and the relationship is more extensive than in a traditional primary care practice. The cap protects the depth of every member relationship.
-
The membership runs on an annual contract. You can cancel anytime with 30 days' notice if it is not the right fit. Contracts renew automatically each year unless you let us know otherwise. That said, longevity medicine compounds — the deepest value usually shows up in year two and beyond, when we are working off your real picture and adjusting based on retesting.
